If you are Insulin dependent diabetic you must get the treating provider to complete ITDM 5870within 45 days of your certification exam and bring it to your appointment.
All diabetics must be up to date on DM eye exam, Hgb A1c with in 3 months of certification exam and bring copies of your lab results to your appointment per FMCSA guidelines.
If you are on habit forming medications or stimulants then you must complete a 391.41 Driver Medication Form prior to your certification appointment.
